Small Blessings makes rewards under the Program for referrals submitted by you that both are: (i) accepted by Small Blessings, and (ii) which sign up for child care services. You must provide us with all of the information required in this referral form. We make only one reward for each eligible referral. The specific rewards available under the Program may be changed by us at any time. We may cancel the Program or modify its terms at any time. Small Blessings has the sole right to decide whether to accept or conduct business with a referral. Without limiting our right not to accept referrals for any reason, you will not receive a reward if Small Blessings has identified your referral as a potential customer before receipt of this form.
